AZCERT currently offers the following Annual* Subscription Plans for use of its technology under the terms of use defined in the AZCERT End User Subscription Agreement appended to this order:

License to Embed QTdrugs Technology in Commercial Products

QTdrugs™
Level 1		 Use of AZCERT’s QTdrugs™ list in one or more of the Licensee’s commercial products such as drug databases or decision support programs without the use of the QTdrugs, AZCERT or CredibleMeds® brand in technical or promotional materials. $20,000 annual fee, plus API Fee, if applicable (below)
QTdrugs™
Level 2		 Level 1 benefits, but with approved attribution to CredibleMeds® name/logo/brand and QTdrugs™ in communications and/or marketing. $25,000 annual fee, plus API Fee, if applicable
QTdrugs™
Level 3		 Level 2 benefits, plus API access to QTdrugs supplemental data now provided at CredibleMeds in popups for each drug, including commonly used brand names, main therapeutic uses, QT-relevant routes of admin, market status, FDA label info such as mention of QT increase or TdP, ECG recommendations, Warnings for cLQTS, contraindicated concomitant medicines and QT-relevant clinical conditions for conditional risk drugs) $30,000 annual fee, plus API Fee

The following API fees for the above Annual Subscription Plans are billed quarterly, pro rata:

QTdrugs™ API Fee
(Level 1, Level 2, Level 3) 		Fee for API “accession”, “call” or “ping” per year:

$1,000 for API ping 1 through 1,000
$0.20 per API ping for pings 1,001 through 10,000
$0.05 per API ping for pings 10,001 through 100,000
No additional charge for API pings above 100,000

Additional access billed as above

Subscription orders will be invoiced when initially approved and annually by AZCERT. Subscription fees are due to be paid within 30 days of invoice. API fees (if applicable) will be billed pro rata on a quarterly basis. All payments should be made to:
